Medicare Advantage: grievance or coverage appeal?
A complaint about plan operations or staff conduct differs from a request to reverse a coverage decision. The correct process depends on what happened and which notice the enrollee received.
Operations or conduct concern: grievance
Enrollee dissatisfaction with a Medicare health plan’s operations, activities or behavior other than an organization determination, such as appointment-access or staff-conduct concerns. Not an appeal seeking reversal of a benefit/coverage denial, an independent provider reimbursement dispute, Part D grievance or hospital discharge appeal. Clinical quality-of-care findings and special expedited handling are not determined here.

Adverse organization determination: reconsideration
Enrollee-side appeal of a Medicare Advantage adverse organization determination for an item or service; a physician may act as the official source permits. Not an independent provider reimbursement/contract dispute, an Original Medicare MAC appeal, Part D drug appeal, grievance, hospital discharge appeal or IRE filing. Expedited clinical qualification and a late-filing extension are not determined here.

Keep filing and response clocks separate
The time allowed to file the enrollee’s request differs from the plan’s response period. Special expedited grievances, quality-of-care complaints and discharge/termination reviews have additional rules. This comparison does not decide clinical urgency, assess care quality, calculate an individual deadline or replace the actual plan notice.
For research, identify the event or notice, its relevant date, the enrollee/requester role and the plan’s stated channel. Keep submission evidence in your own records; do not send patient documents to IntelClaim.
