Retrospective hospital-status appeal after the ordinary filing period ended
Original Medicare beneficiaries, their appointed/authorized representatives, estate filers or people legally authorized for a deceased beneficiary. CMS-10885 excludes providers, suppliers, state Medicaid agencies and other insurers from representing the beneficiary, and excludes Medicare Advantage enrollment at hospitalization. External eligibility review must confirm: inpatient admission on/after January 1, 2009 changed to outpatient observation; observation after that change; the relevant MSN or MOON; and a first appeal or a prior final decision after September 4, 2011. The form also requires at least one additional condition: outpatient coverage denied because the person lacked Part B, or a hospital stay of at least three consecutive days with fewer than three inpatient days followed by SNF admission within 30 days. Q2 determines eligibility. Not the fast/prospective CMS-10868 process for current hospital patients or an ordinary provider claim appeal. No coverage, refund or good-cause conclusion is made.

Virginia DMAS provider informal appeal: identify the adverse-action class
Administrative source reference only, not clinical approval or a decision that a provider may appeal a particular matter. Confirm an actual appealable DMAS decision, necessary prior reconsideration/contractor review, and representative authorization; early filing without appeal rights or failure to exhaust can be dismissed. ASCQR extraordinary-circumstances reporting exception or extension request
Ambulatory surgical center requesting an exception or additional reporting time under the Medicare ASCQR program after an extraordinary circumstance affected its ability to comply with specified reporting requirements. This is not the ASC prior-authorization demonstration, an appeal of a claim denial, an APU reconsideration, a clinical-quality judgment or a determination that a facility qualifies for payment relief. A request may identify multiple requirements/reporting periods, but each must be specified. CMS decides case by case; submitting a request does not guarantee full or partial approval.

Both state60 calendar days from the extraordinary circumstance; the label difference is retained and does not establish a separate effective date. Public-file monitoring resolves the explicitly observed QualityNet metadata mapping and hashes the PDF, excluding temporary download locations from saved evidence.
Hospital IQR reporting exception: non-eCQM event window and eCQM April 1 rule
Hospital participating in the Medicare Hospital Inpatient Quality Reporting program whose ability to satisfy specified reporting requirements was affected by an extraordinary circumstance beyond its control. Identify each affected program requirement and reporting period. This route is separate from an APU reconsideration, a Medicare claim appeal, Hospital OQR, ASCQR and the Medicare Promoting Interoperability hardship process. CMS reviews requests case by case; filing does not guarantee complete or partial approval. Regional or systemic circumstances do not establish a blanket exception without an actual CMS grant. Clinical measure performance, validation results and payment eligibility are not assessed here.

IPFQR FY2027 annual payment update reconsideration after notification receipt
A facility subject to the Medicare IPFQR Program disputing an APU reporting determination affecting FY2027. CMS identifies facilities paid under IPF PPS, including psychiatric hospitals and qualifying psychiatric units in acute care or critical access hospitals. The actual notification must identify the unmet reporting requirements and the facility must explain why it believes those requirements were met. This is administrative reconsideration, not a psychiatric diagnosis/treatment decision, a clinical measure-validation adjudication, a claim denial appeal, IQR/OQR/ASCQR review or an ECE request. A later PRRB appeal is a separate procedure requiring a reconsideration and adverse decision; no PRRB filing date or payment entitlement is calculated.

ESRD QIP ECE change request: under review versus already approved
A dialysis facility with an existing ESRD QIP ECE request seeking to correct submitted information or revise its estimated end date. The FAQ distinguishes under-review support-mediated changes from the new request required after approval. Existing Corporate Point of Contact or Facility Point of Contact QIP roles provide UI access for facilities within their purview. Email-submitted requests can appear in the UI because CMS Support enters them; that is not a second independent application. A CPOC can group facilities only when their start and end dates match; differing event dates require separate requests. This is separate from a score-preview inquiry, a merits appeal, initial ECE eligibility, a clinical performance calculation or withdrawing a CMS-granted blanket exception.

South Dakota Medicaid provider fair hearing after claim reconsideration
A provider challenging the decision on a South Dakota Medicaid claim reconsideration. This does not turn a general request to change coverage, service criteria or the fee schedule into an appeal: the manual states those coverage/fee-schedule request decisions are final and cannot be appealed. This entry also does not establish a member-benefit, MCO contract, provider-enrollment or every sanction appeal route, and does not decide clinical necessity or reimbursement entitlement. Wisconsin ForwardHealth provider appeal after HMO or PIHP payment dispute
Contracted BadgerCare Plus/Medicaid SSI HMO and Children's Specialty Managed Care PIHP provider payment disputes, for enrollees eligible for and enrolled in that HMO/PIHP on the date of service. ForwardHealth will return an appeal not first made to the plan. Out-of-network and non-contracted provider disputes can have different rules and are outside this narrow entry. Alabama pharmacy complaint about PBM reimbursement for eligible paid commercial claims
Pharmacies or pharmacists alleging PBM reimbursement noncompliance under Alabama state PBM laws for eligible commercial/nonfederal claims. Excluded: Medicaid, Medicare PartsB/D, Medicare Advantage, FEHB, Tricare, federally governed hospice and other federally governed plans. PEEHIP/State Teachers and Retired Teachers group is excluded from the reimbursement portion under the guide. The Medicaid reimbursement rate is a statutory comparison input for eligible commercial claims; it does NOT make Medicaid claims eligible for this complaint route. General Consumer Services cannot assist medical providers, and the pharmacy PBM form must not be replaced with a consumer complaint form. Alaska provider insurance complaint or inquiry to the Division of Insurance
Provider/facility/clinic complaint or inquiry to the Alaska Division of Insurance. This entry uses the provider-specific form and covers administrative health-insurance complaint intake; other policy types printed on the form are outside this narrow health entry. The form asks whether a plan is self-funded and explains that self-funded or other plans may fall outside the Division jurisdiction. Signature authorizes forwarding the complaint and documentation to the applicable federal/state authority; it does not establish Alaska jurisdiction over every plan or entitlement to payment. This is not a consumer complaint portal, Medicaid provider fair hearing, external medical-necessity review or legal/clinical judgment.
Filing window: No filing deadline, tolling effect, postmark-versus-receipt rule or filing-hour/timezone cutoff is established by this provider form. Do not treat this regulatory complaint/inquiry as extending an insurer appeal, contract, arbitration or court deadline. The form requests copies of all appeals but does not independently establish exhaustion or a universal prior-appeal waiting period.. No personalized deadline is calculated.
Payer: Alaska Division of Insurance. State scope: Alaska; provider/facility/clinic complaint or inquiry concerning health insurance; self-funded and other outside-jurisdiction plans may be forwarded.
Postal address: Alaska Division of Insurance, 550 W. 7th Avenue, Suite 1560, Anchorage, Alaska 99501-3567. Use the actual notice and the process-specific official submission instructions.
Mail the completed provider form and supporting documentation to the page-one Division address, or email consumerservices@alaska.gov as explicitly directed on page2. Provide provider/facility and contact details, patient/policyholder and employer/plan identifiers, self-funded status (Yes/No/Unknown), insurer or claims administrator, policy effective date, claim number, service date and date claim submitted. Answer the form questions about noncoverage, facility network status and emergency services; the product does not decide those clinical/contract facts. A factual statement is required; enclose the claim, all written correspondence and copies of all appeals. Sign, date and supply printed name/title for authorization to forward. Do not omit the employer solely because the insurer brand is known: the form says employer information helps determine regulatory authority. Printed phone907-269-7900 and fax907-269-7910 are contact details, but the provider submission instruction explicitly specifies mail/email, so no fax filing route or verified direct portal is certified. No PHI is collected, email sent, complaint submitted, representative appointed or account created by this product.

Florida workers compensation provider complaint for an unadjudicated medical bill
Florida workers compensation medical provider with an accurately completed bill for authorized treatment/services that the carrier has failed to adjudicate through payment/EOBR or a Notice of Denial. It is not a generic commercial-health, HMO, Medicaid, Medicare or patient appeal. If the carrier has issued an EOBR disallowing or adjusting payment, the FAQ describes a DIFFERENT Petition for Resolution of Reimbursement Dispute, including a60day receipt-based filing period and service requirements; this silent-bill complaint does not replace or extend that petition deadline. Whether treatment is authorized, a claim compensable or reimbursement owed must be determined separately; no clinical merits or exact amount is decided here.

Georgia managed care provider claim complaint to OCI Consumer Services
Health care providers with complex claim problems involving managed care insurers regulated by Georgia OCI. The general eligibility page excludes self-insured employers and health/welfare or union plans, federal employee health/life insurance, Medicare HMOs, military insurance, Medicare, Medicaid, State of Georgia Employee Health Plan and University System of Georgia. Policies purchased in another state are generally excluded, but the agency notes HMO policies may be an exception and directs callers to OCI for assistance; no individual jurisdiction determination is computed. Illinois provider health insurance claim complaint to the Office of Consumer Health Insurance
Provider health-insurance/HMO claim delays, denials or unsatisfactory payment within IDOI authority. Complaints by collection agencies that BOUGHT the claims are not accepted. The agency excludes self-insured employer/health-welfare plans, Medicare/Medicare HMOs, military insurance, Medicaid, workers compensation and listed state programs; other-state policies are generally excluded with an HMO-policy exception requiring jurisdiction review. The agency generally stays out of provider-contract disputes, while noting possible assistance for balance billing, assignment, recoupment or prompt payment. Medical-necessity/coding/usual-charge disputes involve limited agency authority; this product makes no clinical, coding, coverage, contract or payment-entitlement determination.

Indiana IDOI provider claim complaint after unresolved insurer contact
Provider health-insurance complaints involving companies licensed in Indiana and policies within Indiana jurisdiction; the agency directs other-state policy complaints to the appropriate state. Self-funded employer plans, Medicare and Medicaid are excluded; workers compensation goes to the Workers Compensation Board subject to the agency stated award qualification. Claim monetary amount must be AT LEAST250USD for healthcare-provider claims or AT LEAST5000USD for hospital claims; lower amounts are returned. Missouri DCI provider claim and prompt-payment complaint
Administrative provider assistance with delayed, denied or unsatisfactorily settled health claims involving Missouri-regulated fully insured insurers/HMOs. Excludes self-insured employers/health-welfare plans, federal employee plans, military insurance, Medicare including Medicare HMOs, MO HealthNet Medicaid, Missouri state employee plans, workers compensation and policies purchased in another state. An insurer acting as an administrator does not establish jurisdiction over a self-funded employer plan. The form includes a third-party-administrator checkbox, but that checkbox does not override the webpage jurisdiction exclusions. DCI does not negotiate private provider contracts, though it may assist assignment, recoupment or prompt-payment matters within its legal authority. Oregon DFR medical provider complaint: temporary email filing route
Medical providers are explicitly directed away from the general consumer online insurance complaint form and toward HCProvider.Complaints@dcbs.oregon.gov. The separate provider form page confirms that the form is temporarily out of order and directs providers to email complaint details while it is repaired. This record identifies that official administrative routing instruction only. The two pages do not establish specific fully-insured, self-funded, Medicare, Medicaid, employer-plan or out-of-state eligibility, independent legal standing for a particular dispute, mandatory exhaustion or guaranteed jurisdiction. Confirm these with DFR before relying on this route. Texas TDI provider and billing-service health claim complaint
Providers and their third-party billing services may seek help with health claim payments. TDI regulates HMO and fully insured health plans; the page says these represent less than a quarter of Texas health coverage. Its screening instruction is to complain to TDI if the front of the patient ID card says TDI or DOI, or the matter concerns IDR; this product does not decide individual regulatory jurisdiction. TDI also regulates Employees Retirement System of Texas (ERS) and Teacher Retirement System of Texas (TRS) matters related to CERTAIN out-of-network claims THROUGH the IDR program. This is not general ERS/TRS jurisdiction over every claim or an instruction to file a new IDR request through this complaint record. Complaints concerning IDR requests and settlement outcomes are distinct from an arbitration request or an award. Other health-plan types require the appropriate agency; no automatic Medicare, Medicaid or self-funded-plan inclusion is asserted. This record does not address workers compensation, TMHP Medicaid claim appeal, clinical necessity, coding or payment entitlement.
Filing window: The reviewed source does not establish a maximum agency complaint filing deadline, start trigger, agency filing fee or tolling rule. It asks for documentation showing all appeals have been exhausted; this is not evidence of a specific insurer appeal deadline or a universal extension. For IDR participation or settlement complaints, retain efforts to resolve the matter directly with the health plan. A closed complaint can be started again after the required information is obtained, but the page does NOT say this preserves any insurer, contractual, court or statutory deadline. No individual due date is computed.. No personalized deadline is calculated.
Payer: Texas Department of Insurance. State scope: Texas TDI-regulated HMO/fully insured health plans; certain ERS/TRS out-of-network IDR matters separately qualified.
Postal address: not verified. Use the actual notice and the process-specific official submission instructions.
Use the online complaint system linked on the official provider source page. For IDR participation/settlement complaints include direct resolution efforts with the health plan. The agency says the ID card and requested material MUST be uploaded; without enough information TDI will close the complaint, and it permits starting a new complaint once necessary information is obtained. For MORE THANfive claims, call800-252-3439 or IDR help855-839-2427 for advice; five is not stated as an absolute filing cap or fee. No provider postal/fax/email filing route is established by this page. This research product collects no patient card, PHI or actual case material, sends no message, submits no complaint, creates no account, accesses no licensed code set and pays no money.

Washington OIC medical-provider health insurance complaint and conditional patient release
The provider-specific OIC guidance covers medical providers helping patients file health-insurance complaints, and the linked provider-rights guidance also permits providers to ask OIC to review concerns with their insurer. If a complaint does not involve a health-insurance claim, the agency says it cannot help obtain payment for services. This is not a guaranteed collection service, contract reimbursement award, insurer appeal, external medical review or provider disciplinary complaint. State health-plan requirements apply to plans the OIC regulates; the guidance excludes programs it does not regulate including Medicare, Apple Health and TRICARE and lists employer-funded health plans among coverage not treated as health plans by these state requirements. It does not establish jurisdiction over a particular plan, compel an insurer to contract with a particular provider, determine reimbursement rates, or adjudicate clinical necessity.

A provider may act as the covered person's representative; this is not an independent provider contract-rate or collection dispute. Alaska workers’ compensation medical provider: initial claim form and Board mailing address
Only the initial administrative Claim for Workers’ Compensation Benefits (Form 07-6106) for unpaid benefits the claimant believes are due. The Division’s official form definitions expressly identify a medical provider as one type of applicant taking this first step toward a Board hearing. The form says to file only after the employer has reported the injury to the Division; if the employer refuses or is unavailable, contact the Division. This does not determine that the injury, treatment, amount, provider standing in a particular case, or requested benefit is compensable. It is not a member health-plan appeal, prior-authorization decision, fee calculation, emergency filing, or a completed hearing request.
Filing window: This form and the current form-definition page do not state an initial provider filing deadline. This record does not establish that there is no deadline. Check the actual denial/reduction and applicable statute/rule promptly. No statutory 60 day appeal or 180 day bill-submission period is imported from a fee-schedule excerpt or an unverified statute endpoint. No individual due date, extension or time zone is calculated.. Georgia WC-206 provider party-interest request: Board address and limits
This is a request to be named a party at interest in a pending Georgia workers’ compensation claim, limited to the roles described in Rule 206 and the form: a claim party, group insurer, or health care provider that covered treatment costs or provided medical services. Reimbursement is sought if workers’ compensation liability is established; filing the form does not establish liability or guarantee payment. The Board provides notice of any hearing where the requesting party may present evidence of its claimed interest. This entry does not itself establish an independent right to schedule a hearing or mediation. Payment peer-review under Rule 203 and clinical treatment authorization under Rule 205 are separate processes. WC-PMT(b), concerning an employee’s failure to attend an authorized appointment and potential disability-benefit suspension, is excluded.
Source verification required. Timing and submission guidance are withheld because the source changed, expired or monitoring is unavailable. Check the official source and actual notice.

The current official WC-206 is Revision 7/2021. The official 2026 Rules cover says changes are enforced October 15,2026; selected Rule 206(a)-(c) was reviewed in that compilation. This does not establish that Rule 206 itself was newly amended on that date or determine application to an older case.
Idaho provider disputed-charge motion: filing, service and separate 30/90-day triggers
Only the Rule 19 process for whether a provider’s medical-service charge is acceptable under the regulation applicable when incurred. The provider must first have complied with the preliminary dispute procedures in IDAPA 17.01.01.803.06; this entry does not certify that those steps were completed in a particular case. Disputes about the reasonableness, necessity or appropriateness of treatment are outside this process and may require a separate Complaint. This is not a prior-authorization request, a change-of-physician petition, a clinical decision, or a determination of an amount owed. The rule states that it stands alone and does not incorporate other Judicial Rules by reference. Reconsideration and later litigation are separate stages. Verified preliminary-rule context: IDAPA803.06 requires applicable billing, preliminary-objection/clarification, provider-reply and final-objection steps. Its periods do not begin before the legally required Notice of Injury/Claim is filed with the Employer; that wording applies to periods in that rule and is not automatically treated as tolling the standalone JRP19 deadlines. A requested report may delay specified payor periods. A provider’s written reply, if any, is due within30 calendar days of receiving each timely preliminary objection or clarification request; missing an objection reply and missing a clarification reply have different consequences. A payor pays or sends its final objection within30 calendar days after receiving the reply. The12month billing provision has listed and nonexhaustive exceptions; no automatic forfeiture, balance-billing permission or individual compliance decision is made.
Filing window: Rule 19(E)(1)(a) permits a provider motion after receipt of a final objection or after 45 days have passed from the date the provider sent the bill without a payor response. The request must be FILED with the Commission AND SERVED on the payor within 30 CALENDAR days from the provider’s receipt of the final objection; if the payor has not responded, the separate limit is 90 days from the date the provider sent the bill. The 90 day clause does not itself say calendar days. The 45 day threshold is not the last filing date. A payor response has a separate 21 CALENDAR day receipt-of-motion period. The rule warns of a bar for untimely requests involving a final objection. No individual due date, tolling, weekend/holiday extension or time zone is computed; check the actual bill, final objection and current instructions promptly. Do not replace the JRP filing periods with these preliminary payor/provider response periods. Verify the actual injury notice, requested-report receipt, objection, clarification and reply chronology. The source includes Commission discretion over billing timeliness; this site does not determine whether an exception applies.. No personalized deadline is calculated.

An emailed Commission filing is an original and does not require another original copy. Filing with the Commission is separate from service: send the motion and supporting documents to the surety through its adjusting office or designated in-state Claims Administrator by mail, fax or personal delivery. Include the actual service destination on the certificate; fax service also requires the fax transmittal confirmation. The form’s email checkbox does not override Rule 19’s stated methods for serving the other party. Use Appendix 6 A, identify the provider/payor, patient, service dates and disputed amount, and attach AppendixA itemization and evidence supporting the disputed charge. Sign/date the provider-or-agent and service sections truthfully. This site collects no personal identifiers, medical records or actual claim; performs no filing/service, fee calculation or clinical review; and hides guidance if either source changes or becomes stale/blocked.

New Mexico workers’ compensation provider: initial MCC billing-dispute request and mailing address
Initial administrative Request for Billing Dispute Review to the MCC Bureau, using the currently linked approved form. Rule 11 A permits any party to request a director’s determination of a billing dispute. The form offers separate no-response and denied/partial-payment branches. It does not decide compensability, clinical necessity, the amount payable, authorization, insurer jurisdiction or provider standing in an individual case. Utilization-review and case-management disputes use separate forms and are outside this record. The Bureau first attempts informal resolution; unresolved matters may produce a separate clerk notice. Oregon workers’ compensation provider: initial non-MCO medical-fee review and mailing address
Initial administrative request for director review of a medical-fee or compensable-bill payment dispute, limited to the non-MCO/non-MCO-action branch. Use both Forms 2842 and 2842 a for medical-fee disputes. A party need not be represented during director administrative review; that does not determine representation requirements in a later proceeding. Questions requiring a determination of condition compensability or sufficient causal relationship are transferred to the Board under 0008(1)(b). This record does not decide clinical necessity, treatment authorization, accepted conditions, causation, claim coverage, amount payable or a hearing result. It does not cover MCO dispute exhaustion or later reconsideration/hearing filing.
Filing window: For claims not enrolled in an MCO, or disputes not involving an MCO action or decision, OAR 436-009-0008(2)(c)(B) requires a medical provider request within 90 days of the mailing date of the most recent EOB or similar notification received regarding the disputed service or fee. Unchanged rebilling does not restart that period. This provider trigger is distinct from the worker’s knowledge trigger and the insurer’s bill-action-due trigger. Worksheet 2842 a’s generic knowledge wording does not replace the more specific provider rule. For nonpayment/reduction, the worksheet instructs the requestor to contact the insurer first, provide its explanation and wait at least 45 days from insurer receipt of the bill unless denial or reduced payment has already been received. No individual due date, time zone or extension is calculated. MCO 30/60-day processes and exceptions require separate review and are outside this record.. No personalized deadline is calculated.
Payer: Oregon Workers’ Compensation Division — Medical Resolution Team. State scope: Oregon workers’ compensation; non-MCO claims or disputes not involving an MCO action or decision only.
Postal address: Workers’ Compensation Division, Medical Resolution Team, 350 Winter St. NE, P.O. Box 14480, Salem, OR 97309-0405. Use the actual notice and the process-specific official submission instructions.

Provide the specific dispute, service dates, relief sought, claim and party contacts, original bills, relevant records, bill analyses and correspondence; list each disputed billing code, service date, amount billed and amount paid on 2842 a, adding sheets if needed. Billing-code fields do not supply a licensed code database. Simultaneously notify all interested parties and representatives and provide copies of the request and evidence free of charge. Where relevant, disclose a known causation/compensability issue and attach MCO-review documents; insurer certification is required only for an insurer’s request. Noncompliance can lead to dismissal. This site collects no patient records, performs no notice service and submits no request. Minnesota workers’ compensation provider: initial amount-only Medical Request and DLI address
Initial DLI Medical Request (MQ03) for a non-hospital, non-ASC health care provider disputing an insurer’s denial of its billed charge as excessive in amount, limited to an already accepted workers’ compensation claim with no certified managed-care dispute. Current Rule 1415.3800 permits provider requests only for payment denials based on excessiveness under section 176.136 subdivision 2; that statute defines excessiveness more broadly than numerical amounts, but this record covers only the amount branch. It does not decide reasonable treatment, necessity, diagnosis, causation, authorization, fee-schedule coding, compensability, provider credentials, balance-billing rights or reimbursement. Do not use this route for an entire-claim primary-liability denial or a dispute about whether the work injury caused the need for treatment. Certified managed-care exhaustion, hospital/ASC procedures, employee claims, insurer responses and a formal de novo hearing require separate review.
Filing window: Before filing MQ03, contact the insurer or DLI Alternative Dispute Resolution Unit. The form says not to submit unless the insurer has had 30 days to review the bills or has already refused payment; this is a review prerequisite with an earlier-refusal exception, not a general limitation period or a new 30-day appeal deadline. The form does not establish a standalone initial filing deadline for this narrowly defined provider request. Do not interpret that silence as unlimited time, or assume contact, mediation, rebilling or filing MQ03 tolls or extends another deadline. Different hospital, ambulatory surgical center, managed-care and later-hearing timelines are outside this record. No individual due date, time zone, fee, interest or amount is calculated.. No personalized deadline is calculated.
Payer: Minnesota Department of Labor and Industry — Workers’ Compensation Division. State scope: Minnesota workers’ compensation; non-hospital, non-ASC providers; initial liability accepted; no certified managed-care dispute; amount-only excessive-charge denial.
Postal address: Minnesota Department of Labor and Industry, Workers’ Compensation Division, 443 Lafayette Road N., St. Paul, MN 55155. Use the actual notice and the process-specific official submission instructions.
The current-linked form directs the completed Medical Request and attachments to DLI at the address above, also corroborated by DLI’s current St. Paul office listing. File initially with DLI, not directly with the Office of Administrative Hearings. Include claim and party details, the specific disputed charges and relief sought, evidence of the insurer’s excessive-charge denial, itemized bills and the appropriate supporting record. Complete the other-payer section even if no other payer paid; identify potential intervenors and serve all required parties with the request and attachments, recording the copy-service date and signing the form. Provider standing and applicable notice/service requirements must be checked for the actual case. The PDF mentions fax but supplies no fax number in its destination block; no fax or filing email is invented. Worker-specific Campus instructions do not establish an independent provider account’s filing permission, so no provider portal route is verified here. North Carolina workers’ compensation provider: informal fee-dispute assistance after Form 26I
Initial informal Medical Fees Section assistance for an unpaid provider bill after the direct Form 26 I inquiry step. A provider completes questionnaire SectionsA–C and sends them with the disputed bill materials to the actual employer/carrier; that initial questionnaire is not filed with the Commission. If carrier identity is unknown, use the Commission’s public coverage-search instructions for the employer and injury date or request limited contact-identification help. The agency does not disclose private claim information or facilitate provider-payer communications at that initial lookup stage. Informal assistance is distinct from a Motion to Intervene, an order granting limited standing, Form 33 I, a hearing, a fee award or legal representation. The Medical Fees Section expressly cannot authorize or approve treatment and does not reimburse an employee’s out-of-pocket expenses or mileage. Texas workers’ compensation provider: request a BRC to review an MFDR decision
A health care provider with an existing Texas DWC Medical Fee Dispute Resolution decision may use Form-045M to request a benefit review conference concerning that fee decision. This entry covers the initial request to schedule that BRC, for the non-network or approved out-of-network payment-dispute setting; it does not create a provider’s standing, establish compensability or decide whether a service was medically necessary. It is not an initial MFDR request, a network clinical appeal, a SOAH hearing request or judicial review. Unresolved compensability, extent, liability or medical-necessity questions require their own procedures. The original explanation of review and bill must be available. CCR9792.5.5(c)(1)(A) permits requesting second review on the initially reviewed bill, using the prescribed condition qualifier and code. This entry uses that specific alternative. Dental, pharmacy, electronic bills and medical-legal bills have different methods and are excluded. It does not decide compensability, medical necessity, authorization, correct coding, a contract rate or the amount legally owed. It is an initial claims-administrator review, not a complete IBR, lien or court route, and not a clinical approval or payment guarantee.

New Mexico MCO provider appeal of its own payment denial
A provider or its authorized representative disputing an intended or taken adverse action based on the MCO denial of the provider own payment. This is the provider payment route; acting as a member authorized provider requires its separate designation and member appeal procedure. A non-contracted provider receiving instructions about member delegation is not automatically appointed to represent the member. This entry is limited to a Rhode Island provider alleging a prompt-processing violation involving a fully insured commercial health plan, subject to OHIC actual standing and jurisdiction review. Virginia BOI provider complaint about possible carrier payment-practice violations
Provider complaints about a possible pattern of health-carrier violations of ethics/fairness practices with contracted providers under38.2-3407.15, or another insurance law, regulation or Commission order under38.2-237, subject to actual BOI oversight. The packet describes investigation and affected Virginia fully insured claims; it does not establish jurisdiction over a particular self-funded, Medicare, Medicaid, federal employee, military or workers-compensation case. This entry is administrative regulatory research, not a member medical external review or a ruling that a claim is overdue. SubsectionL says the Commission has no jurisdiction to adjudicate individual controversies arising out of this section; no individual contract judgment, payment award or guaranteed recovery is promised.
Filing window: For the selected prompt-payment complaint under38.2-3407.15(C), first make a reasonable effort to confer with the carrier contact listed in the provider contract and provide enough supporting documentation to identify the disputed claims. At least30calendar days generally must pass from the request to confer if the carrier has been responsive; the statute preserves a provider-judgment exception when the carrier has not been responsive. This is a prerequisite, not a maximum BOI complaint filing deadline. The June2024 provider PDF instead refers to the date of the provider contract; that wording is inconsistent with the current primary statute request trigger and must not drive a filing-date calculation. Confirm the applicable prerequisite and form attestation with BOI; no individual due date, maximum filing period, fee or interest amount is calculated.. No personalized deadline is calculated.

Ohio ODM provider administrative reconsideration after an eligible department action
Medicaid providers or specified persons contesting an ODM administrative action that is eligible for paragraphD reconsideration, subject to another Ohio Administrative Code provision. ParagraphC lists actions outside Chapter119 adjudication, including specified claims-payment denials and adjustments; this does not mean every claim denial is automatically eligible for this route. ParagraphB Chapter119 adjudication is a different procedure. ParagraphA excludes specified managed-care contracting and statutory proceedings. Beneficiary benefit hearings, managed-care plan claims disputes, hospital-specific and nursing-facility-specific procedures must be evaluated separately; no provider standing, medical necessity or payment entitlement is decided.
Filing window: There is no universal fixed30day filing period established by paragraphD. If ODM sets a deadline for this reconsideration, it must be no fewer than30 days from the date appearing on the letter sent to the person. The rule does not specify calendar or business days for this minimum. Use the actual ODM letter and applicable specific rule: this is a minimum permitted notice window, not a personal due date, not a receipt-date trigger and not an automatic30day extension. Late requests and supporting documents may be considered at the department's discretion; acceptance is not guaranteed.. No personalized deadline is calculated.

ParagraphD permits the director, assistant director or responsible deputy director to designate a reviewer who was not involved in the original decision. ODM will not reconsider its reconsideration decisions under this paragraph. This does not determine whether a different judicial or other remedy exists. Payment withholding, audit amounts, fault, individual deadlines and clinical judgments are outside this record. NPWest DMEPOS supplier enrollment appeals and rebuttals: Palmetto GBA
DMEPOS supplier enrollment appeals and rebuttals under the NPWest National Provider Enrollment jurisdiction listed above. This is a supplier enrollment route, not a DME claim payment redetermination, a Medicare beneficiary appeal, all Part B provider enrollment matters or a coverage/medical-necessity determination. Use the supplier location and actual determination to verify the responsible enrollment jurisdiction; the supplier jurisdiction is not automatically the DME claims MAC jurisdiction. No individual right to appeal, corrective action plan eligibility, representative authority, reinstatement or payment entitlement is decided.

Iowa Medicaid hearing after denial of prereview reconsideration
An aggrieved party following a denial of reconsideration for the specified preadmission or preprocedure review under441—79.11 or79.12. The provider-facing research route concerns physicians, hospitals or facilities within those specified proceedings; it does not confer standing solely because someone bills Medicaid or represents a member.441—2506 requires a right to a hearing, an ongoing specific personal interest, exhaustion of other available remedies and other applicable requirements. It is not every prior-authorization denial, an ordinary unpaid claim, an MCP postpayment audit, a managed-care member adverse-benefit appeal or a procurement-vendor appeal. Medical necessity, procedure coverage, payment entitlement and the individual right to a hearing remain undecided.
