Medicare Part D: which appeal stage and destination?
Start with the title and issuer of the actual decision notice. A coverage determination denial is followed by a request to the plan sponsor; an unfavorable plan redetermination is followed by independent reconsideration. These are enrollee-side rights, with representative and prescriber roles described by CMS.
After a coverage determination: plan redetermination
Enrollee-side Part D appeal after a wholly or partly denied coverage determination. Not a pharmacy contract/payment dispute, Part C appeal, Original Medicare redetermination, late-enrollment-penalty review or second-level IRE reconsideration. Request urgency, clinical support and late-filing exceptions require separate review.

After a plan redetermination: IRE reconsideration
Enrollee-side Part D merits reconsideration after an unfavorable or partly favorable plan redetermination. Not a first-level plan request, pharmacy contract dispute, Part C IRE case, Original Medicare QIC appeal, dismissal-review deadline or OMHA hearing. At-risk drug-management cases may have automatic referral; confirm the notice rather than assuming a new manual filing is always needed.

Operations or conduct complaint: grievance
Enrollee dissatisfaction with Part D plan operations, activities or behavior other than a coverage determination, such as customer-service problems or a complaint about refusal to expedite a request. Not an appeal to reverse a drug coverage decision, a pharmacy contract dispute, Part C grievance, or independent prescriber grievance right. Clinical urgency and representative sufficiency require separate review.
Source verification required. Filing window and destination are withheld because this source changed, expired or is unavailable.

Source does not establish a universal effective date; current guidance checked, not historical reconstruction
A grievance concerns plan operations or conduct. It does not replace an appeal seeking reversal of a coverage decision. Its event-based filing window differs from notice-based appeal filing and the plan’s response period.
Prepare a research handoff
Keep the coverage determination and any redetermination notice, their dates, the requester’s role, representation documentation where applicable, and a record of the submission channel. The official decision and current instructions determine the necessary supporting materials. Do not upload patient records to IntelClaim.
The period allowed to file differs from the time the plan or IRE has to decide. Requests involving an exception, payment, urgency or an at-risk drug-management determination can follow different handling rules. This page does not assess clinical eligibility, calculate a personal deadline or resolve a pharmacy reimbursement dispute.
