Noridian DME appeal mailing addresses: JA and JD
For supplier billing teams researching a first-level Original Medicare claim redetermination. Check the contractor and jurisdiction printed on the actual claim notice before choosing a mailbox.
Choose the jurisdiction and delivery method
JA and JD use different USPS PO boxes. These entries describe the USPS column of Noridian’s mailing table. Its certified/courier column includes a street address and a different ZIP code; do not combine parts from those two columns. Refund checks have separate lockboxes. An address shared by several departments does not make their request types interchangeable.
Noridian DME Jurisdiction A redetermination postal route
Jurisdiction A DME only; match the contractor and jurisdiction on the claim notice. No state eligibility is inferred from the mailing table.
DME supplier first-level redetermination requests handled by Noridian Jurisdiction A. The correspondence row also lists overpayment redetermination/rebuttal and Recovery Auditor redeterminations; this record does not determine recoupment deadlines. Not another jurisdiction, Medicare Advantage, a refund payment, PA request, ADR response, reopening or second-level QIC appeal, even when departments share a PO box.
USPS address
Noridian JA DME; Attn: _________; PO Box 6780; Fargo, ND 58108-6780
The official table leaves the attention/department line blank. Confirm that line on the actual notice and current request instructions before mailing. This entry is the USPS route; certified/courier addresses and refund-check lockboxes differ.

Deadline and request contents are separate checks
