Provider-side party to an actual QIC reconsideration dismissal, or properly appointed representative, requesting OMHA to review correctness of that dismissal. This does not permit review of a QIC’s binding review of a MAC dismissal. Not QIC merits hearing, Medicare Advantage or direct merits adjudication. Annual amount and other filing requirements must be independently met.
